Position Overview:

Quality Angels is looking for an experienced Residential Program Specialist to join our team. The 6400 Residential Program Specialist is responsible for providing case management and direct support services to a caseload of up to 12 clients in a residential setting. This role includes developing, implementing, and managing Individual Support Plans (ISPs), coordinating client services, and ensuring that each client's needs are met in compliance with regulatory requirements. The Program Specialist will also train staff, create activity schedules, develop client menus according to medical directives, and manage all necessary documentation. A key part of the role involves maintaining communication with Support Coordinators and families, and ensuring all required reports and assessments are completed. The ideal candidate will have the ability to work independently, excel as a team player, and possess strong writing and verbal communication skills.

Key Responsibilities:

Case Management:

  • Oversee a caseload of up to 12 clients, ensuring their individualized needs are met.
  • Develop and implement Individual Support Plans (ISPs) for each client, including goal setting and tracking progress.
  • Conduct regular assessments, including initial and annual assessments, and update ISPs based on client progress and changing needs.
  • Maintain accurate and up-to-date client records in compliance with 6400 regulations.

Client Support & Coordination:

  • Schedule and attend monitoring meetings and annual meetings with clients, Support Coordinators, and families.
  • Communicate regularly with clients' Support Coordinators and families regarding progress, changes in care, and any concerns.
  • Schedule and attend client appointments (medical, therapeutic, etc.) as needed, ensuring proper follow-up.
  • Be available for on-call support in case of client emergencies, responding promptly and appropriately.
  • Create and maintain clients' daily and weekly activity schedules, promoting engagement in meaningful activities.
  • Develop client menus that adhere to doctor's orders, dietary restrictions, and individual preferences.

Staff Training & Development:

  • Train new and existing employees on ISPs, client needs, and any changes to care plans.
  • Provide ongoing coaching and support to staff to ensure compliance with regulatory standards and client care protocols.
  • Conduct in-service training sessions to address specific client needs and behavioral management strategies, including annual fire safety training for residents.

Program Management & Compliance:

  • Complete required reports, including monthly reports, quarterly reports, HRST ratings, face sheets, and other necessary documentation.
  • Update annual documentation, including consent forms, room and board agreements, fire department letters, and other required forms.
  • Develop and implement activities that enhance clients’ quality of life and promote independence.
  • Collaborate with interdisciplinary team members, such as therapists, dietitians, and healthcare providers, to coordinate comprehensive care.
  • Ensure the residential environment is safe, supportive, and conducive to clients' well-being.

Compliance & Documentation:

  • Ensure compliance with all 6400 regulations, including documentation, reporting, and record-keeping.
  • Prepare and submit required reports in a timely manner, including progress notes, incident reports, monthly summaries, and other documentation as needed.
  • Monitor and implement quality assurance measures to maintain high standards of care.

Qualifications:

  • Bachelor’s degree in Human Services, Social Work, Psychology, or a related field preferred; or equivalent experience in the field.
  • Minimum of 2 years of experience working with individuals with intellectual and developmental disabilities (IDD).
  • Knowledge of 6400 regulations and experience in residential program management.
  • Strong organizational and time management skills.
  • Excellent writing and verbal communication skills.
  • Ability to work independently and as part of a team, with a collaborative and cooperative attitude.
  • Ability to work flexible hours, including on-call availability for client emergencies.
  • Valid driver’s license and reliable transportation.

Physical Requirements:

  • Ability to lift up to 25 pounds.
  • Ability to perform duties that require frequent standing, walking, bending, and reaching.
  • Ability to manage stressful situations and adapt to changing environments.

Work Environment:

The 6400 Residential Program Specialist will work primarily in a private office or residential setting, with some responsibilities requiring travel to client appointments, meetings, and monitoring sessions. The position requires flexibility in scheduling to ensure that client needs are met, including the potential for evening and weekend hours.
